ONLINE appointment booking

General booking Instructions

Please follow these instructions when booking appointments online to ensure success: 


  • One person per booked appointment (we cannot accommodate add-ons, for example a spouse being seen at the same time, please book one person per appointment slot only) 
  • Please limit to 2 health concerns per visit (if you have more concerns please give our office a call directly so we can accommodate enough time accurately)
  • One appointment per patient per day (do not book more than one appointment per day, if this is seen on the schedule, the latter appointment will be cancelled) 
  • Book under the patient being seen (book under the child, not the parent, if the appointment is for the child)
  • Select the appropriate appointment type (if incorrect, your appointment may need to be rescheduled. Please read about appointment types below BEFORE you book and if you are unsure, call the office)
  • When in doubt - call the office (our staff are always happy to assist you, please do not guess at your booking if you are unsure, please call) 

Appointment Types to Select from

These visits consist of developmental screening, measurements, physical exam, and immunizations. The schedule for well baby visits is by baby’s age and occur at  2 months, 4 months, 6 months, 9 months, 12 months, 15months, 18 months


This appointment type is for a diabetes visit.  Other health concerns or new symptoms/issues cannot be discussed at this appointments as they are dedicated to focus on your diabetes care and management. 


This appointment is for a physical exam and completion of Ministry of Transport form for upgraded liecenses. 


This visit type is to focus on preventive health measures. Previously known as a “Physical” or “Annual Check-Up”, it is now referred to as a Preventive Health Visit or PERIODIC Health Visit, to reflect that it doesn’t need to occur annually and the focus is on prevention. This appointment type is for adults 50 years of age and older. Your physician will review your health history, discuss and order preventive screening tests you are due for, and review and give immunizations you are eligible for. As you can see there is a lot to review at these appointments, if you have any symptoms or new health concerns, you should NOT book this type of appointment. No other issues can be discussed within the time allotted for these types of visits. Please book a separate appointment on another day. 


This appointment type is to discuss  your pregnancy. To review any labs or imaging needed and any referrals to be put in place.


This appointment is for any concerns you have already addressed with the physician and are following up on due to ongoing symptoms/concerns.


Please bring the medical form needed with you to the appointment.


To discuss your current medications and if you are needing to continue or have a change in dosage or stopping medication.


This visit type should be used if  you are needing to see a physician for concerns related to your mental health (for example - anxiety, stress, depression symptoms). 


This appointment is for any new concerns you are having with your health. 


This appointment is to review any medications you are needing renewals on. 

Please book minimum 7-14 business days before you are out of any medication.
